3. Watch for an email from our scheduling platform (Simple Practice) that will have you set up an online portal and complete the necessary paperwork before you come in. 

3. Complete the paperwork before we meet. 

Please contact your insurance company to confirm your benefits and coverage for mental health services. Some plans require prior authorization.

Cash Rate: $225 for first appointment $175 per hour afterwards.

As an ACMHC, ​Dax can currently take the following insurances:

CIGNA
PEHP

For those without these insurance plans or have no insurance, Dax offers a sliding scale rate as well.
